Prescriptions are generally issued by our computer. The left side of the sheet is the usual prescription for the chemist; the right hand side of the prescription sheet may show a list of prescription items, which a doctor has authorised to be repeated. At the bottom of any such list is a review date. This means that you have to make an appointment to have your prescription reviewed on or around that date. Each time you see a doctor, this review date may be extended appropriately.
Repeat prescriptions may be ordered over the telephone from Monday to Friday or by bringing or sending the right hand side of your prescription to the surgery, ticking the items needed.
Prescription Collection
Due to increasing demand and problems of collecting prescriptions late in the afternoon we have revised our prescription system and going forward from Wednesday 18th May 2011.Prescriptions ordered on the day Before will only be issued and ready for collection the next working day not 16.00 p.m. the same day.
Prescriptions ordered on the day After 11.00 a.m. will only be issued and ready for collection two working days later from the date of order.
On Collection Day
Anytime during our surgery opening hours between
08.30 a.m. and 18.00 p.m.
We are closed for lunch between
12.45 p.m. and 13.45 p.m.
Prescriptions will not be given to children under the age of 16 unless they have written authorisation.
If it is difficult to collect your prescription, a local chemist will usually deliver the items for you. Let the surgery know the chemist of your choice, and we will forward the prescription accordingly.
All test results are seen and assessed by the doctor. You may be asked to telephone for some results and the receptionist will be able to pass on the doctor's message. Some complicated tests can only be explained by the doctor, at an appointment. Please note that results for people other than yourself cannot be discussed or given over the telephone.
If you live in our catchment area we will accept you on our practice list. It is practice policy to accept whole families only, who reside at the same address. This enables us to care for individuals within the family and have contact with the family as a whole should the need arise.
If you wish to register at the practice, you should bring your medical card to the surgery. If your do not have a medical card, a form will be provided or download the GMS 1 form here.
To avoid delay in registering with the surgery please complete all relevant boxes and ensure that the form has been signed before posting or bringing in to the surgery.
You will then be registered with the practice not with an individual doctor.
Medical examinations for special purposes including, but not limited to, HGV, PSV, pre-employment, fitness to undertake sports, fitness to travel, are not part of our NHS obligations. If you want the doctor to carry out any of these examinations for you, you should inform the receptionist when making an appointment, as they are performed out of surgery hours. A fee is payable for all non NHS services. There will be a fee levied for the photocopying of results and hospital letters and the completing of forms for insurance purposes, passports and claim forms. If you have any queries regarding what is considered as a non NHS service, please address them to one of our management team who will be happy to assist.
Any patient can book for a routine check with the nurse if they feel that they would like one. Such a screen would include height / weight / blood pressure / urine examination, as well as other tests appropriate to specific individuals.
We have a practice based procedure to deal with
any complaints. Information can be obtained from reception, or you may speak
directly to either the Practice Manager, Mo Pitchforth, or the Assistant
Practice Manager Anne Clayton. We hope that any complaints can be resolved
quickly and satisfactorily, and we are always interested to hear any comments or
suggestions you feel may be helpful to the running of the practice.
